Enhanced Standardized State Formulary Files
The Enhanced Standardized State Formulary Files provide users with detailed information on items covered by individual state Medicaid organizations. Available for every state except Arizona (which does not maintain statewide Medicaid formularies) as well as the District of Columbia, the database allows pre-adjudication of items dispensed to individuals covered under state Medicaid programs.
FEATURES
- Maximum Allowable Cost, when applicable
- Items inactivated by a manufacturer or distributor for 24 months
- Includes item identifiers, type and format codes, billing codes, transaction codes and other fields that are consistent across all state formulary database files
